Coordinated care at home
Healthcare services that come to our home.
We work with visiting healthcare professionals so residents can access many services in familiar surroundings, reducing the need to travel for appointments.
- Ongoing primary care
- Visiting primary care providers work with our home to support residents' long-term primary care needs and coordinate ongoing care.
- Home health & therapy
- We coordinate home health nursing, physical therapy, occupational therapy, and speech therapy based on each resident's clinical needs and eligibility.
- Labs & mobile X-rays
- Laboratory services and mobile X-rays can be arranged at the Adult Family Home when appropriate and available.
- Podiatry & dental care
- Visiting podiatry and dental providers help residents access foot care and oral health services at home.
- Pharmacy delivery
- Our pharmacy delivers residents' medications directly to the Adult Family Home. We coordinate with the pharmacy and prescribing providers to support medication needs.
- Haircut services
- Haircut services can be arranged at our home, helping residents maintain their personal preferences and familiar grooming routines.
Healthcare services are provided by independent professionals and depend on clinical need, provider availability, eligibility, and insurance coverage or private-payment arrangements. Haircut services are subject to availability. These services may involve separate provider charges and are not automatically included in the home's care fees. Some specialty appointments and procedures may still require an outside visit.
